New Patient Intake Forms
All new clients/patients need to fill out the Ancient Arts Intake Forms before their first appointment.

Choose the Fillable option to complete the form in Microsoft Word and email it back to us at [email protected]. Choose the Printable option to print, complete, and scan or fax (206-547-1325) the form back to us. Please aim to return paperwork by the week prior to your pet's initial visit.
If you are unable to access a computer to complete and email us the forms below, you must arrive at least 15 minutes before your appointment time to fill out paperwork and avoid cutting into your pet's allotted appointment time.
The intake form provides a wealth of health information about your pet as well as offers a general idea of why you are seeking acupuncture and holistic treatment.
If new clients/patients arrive at their scheduled appointment time without completed Intake Forms, the time needed to fill out paperwork will take away from time with the doctor.
Please respect our time and the time of fellow clients/patients. We do our best to keep all appointments running on schedule to give clients and patients the time needed with the doctor and support staff.
