Choose the Fillable option to complete the form in Microsoft Word and email it back to us at [email protected]. Choose the Printable option to print, complete, and scan or fax (206-547-1325) the form back to us. Please aim to return paperwork by the week prior to your pet's initial visit.
If you are unable to access a computer to complete and email us the forms below, you must arrive at least 15 minutes before your appointment time to fill out paperwork and avoid cutting into your pet's allotted appointment time.

If new clients/patients arrive at their scheduled appointment time without completed forms, the time needed to fill out paperwork will take away from time with the doctor.
